Goals of Care/POLST

Dr. Barile will introduce a simple Four-Step method for end of life decision making that may be used to structure a conversation or make majort treatment decisions. He will also explain how the NJ POLST form can be introduced using this Four Step model and how POLST may be used as a tool to align personal goals of care with available therapies.
